WELLWATCH REGISTERED GENERAL NURSE c.£25,000 per annum

Wellwatch is an exciting development in the healthcare management of patients with long term conditions run by Central London Healthcare supporting patients in Westminster and Victoria.

The Wellwatch team work with patient’s GP, District nursing and Social Care teams to identify gaps inthe provision of patient’s care and provide and support the patient to stay well for longer.

We are looking for energetic and enthusiastic RGN’s to be part of this programme.

You will have:

• a valid NMC pin card or be awaiting UK registration
• Excellent clinical skills
• Experience working with patients with COPD, Asthma, Depression, Chronic Heart disease, Hypertension, Diabetes & Elderly care
• Experience using a case management system
• Experience coordinating services on behalf of patients
• Good Communication and interpersonal skills
• Ability to work to deadlines and targets
• Experience building relationships and working in a multi-disciplinary team
• A flexible, professional and friendly approach
• Intermediate MS Excel and Word
